Beginning January 2015, our medical plan will be serviced by Aetna Signature PPO. This is exciting, because Aetna is one of the largest and most respected health coverage insurers in the world. 2015 Plan Benefits:

Deductible: $750/$1500
Coinsurance: 80% / 20%
Preventative Care/Screening/Immunizations: Paid at 100%
Primary Care Provider Office: Co-Pay $30
Specialist (Consult/Evaluation): $55
Emergency Room Services: $250
Urgent Care Co-Pay: $30
Most other services offered at $20% coinsurance after deductible has been met (see benefit guide for more details)

New Benefit! Flexible Spending Account

We are excited to offer this new benefit for 2015! The flexible spending account provides us with an opportunity to help pay for health care and dependent care expenses while lowering our taxable income on a pretax basis. This program allows participants to pay for certain IRS approved medical, dental, vision expenses and dependent care expenses. We had an overwhelming number of employees take advantage of the FSA benefit!

Generic Drugs: $5-$10 Co-Pay
Preferred Brand Name Drugs $25-$50 Co-Pay
Non-Preferred Brand Name Drugs $40-$80
Specialty Drugs $20% Coinsurance/maximum $150 per prescription

Dental Policy 2015

We have moved from Delta Dental to Metlife PDP Plus Network
The annual maximum increased from $1,000 to $1,250
Diagnostic & Preventative Services In-Network benefit pays @ 100%
Basic Restorative Services In-Network benefit pays @ 80%
Major Restorative Services In-Network benefit pays @ 50%

Eye Exam In-network $10
Glasses or Contacts $25 Co-pay
Frame allowance – Up to $130 after $25 co-pay
